LUCKY DRAW TERMS & CONDITIONS.

Terms and Conditions for participation in Samsung Galaxy Active Watch Lucky Draw (“Promotion”).

1. This Promotion is organised by HeartVoice Pte Ltd (“HeartVoice”).

2. HeartVoice is offering 5 complimentary Samsung Galaxy Active watches.

3. This Promotion is available only to Eligible Members who have successfully purchased one of the Back On Track! Health Programs sold by HeartVoice.

4. The Promotion is only applicable to participants residing in Singapore.

5. The Promotion is open from 31 October 2020 to 31 March 2020, all dates inclusive (the “Promotion Period”). If the purchase of Back On Track was made earlier than 31st October 2020, the customers will be still entitled to participate in the Draw. Each Lucky Draw will be held on the last week of every month during next 5 months at HeartVoice Pte Ltd, Fusionopolis Symbiosis, 3 Fusionopolis Way #13-25/24, 138633.

The dates of the Draws are 30th November 2020, 29th December 2020, 29th January 2021, 26th February 2021, and 31st March 2021.

6. Each winner is only entitled to win one prize, regardless of how many times their lucky draw coupons are picked. HeartVoice shall be entitled to disregard the second and subsequent picks and re-pick a winner. The chance will be transffered to the next month during the entire campaign duration, and the participant will be only withdrawn from the Lucky Draw untill he/she wins it for the 1st time.

7. HeartVoice shall contact the Winners via email and/or phone within seven (7) days of the conduct of the Lucky Draw (“Notification”).

8. All Lucky Draw participants will be receiving an email from HeartVoice to stay informed about Lucky Draw winners.

9. The prizes are not transferable, exchangeable for cash or any other items.

10. Prizes to be collected at HeartVoice Pte Ltd, Fusionopolis Symbiosis, 3 Fusionopolis Way #13-25/24, 138633, within its operating hours. The Winners are required to redeem their prizes within 14 days from the Notification, failing which, the winner will not be entitled to the prizes.

11. Acceptance of any prize shall constitute consent on the winner’s part to allow the use of the winner’s name, photo, image, voice and/or likeness by HeartVoice for editorial, advertising, promotional, marketing and/or other purposes without further compensation except where prohibited by law.

12. HeartVoice shall not under any circumstances whatsoever be liable for any loss, injury, damage or delay arising out of any cause whatsoever beyond HeartVoice’s reasonable control or any malfunctioning of or defect in or failure of any of the prize or any loss of the use of any prize.

13. By entering the Lucky Draw or accepting the prizes, You agree that no claim relating to such losses or injuries (including special, indirect and consequential losses) shall be asserted against HeartVoice, its affiliates, directors, officers, employees or agents from any and all losses, damages, rights, claims and actions of any kind resulting from the Lucky Draw and acceptance of any prize, including without limitation, personal injuries, death and property damage.
