TERMS AND CONDITIONS FOR BACK ON TRACK PROGRAMME AND COUPONS IN THE BACK ON TRACK PROGRAMME

These terms and conditions apply to the Back on Track Programme (“Programme”) and the Back on Track Programme Coupons (“Coupons“) issued by HeartVoice Pte Ltd (“HeartVoice”) through the purchase of the Programme.

Please do not use the telemedicine service for any emergency medical needs.  Medical emergencies are not part of this service and HeartVoice users are advised to call 995 in the event of an emergency.

A.  The Back on Track Programme

  1. You can only purchase one package in the Back on Track Programme at any one time. To repurchase a package or to buy another package on the Programme, you will only be able to do so after 6 months from your most recent Programme purchase.
  2. By purchasing the Pocket Doctor Programme and the Healthy Starter Programme you allow HeartVoice to release your personal data which will be your name, NRIC, age, nationality, mobile phone and email to The Great Eastern Life Assurance Company Limited.
  3. The Programme package is not transferrable. You commit you are the person registering and are not presenting yourself as someone else or acting as an agent for somebody else By purchasing the Pocket Doctor Programme, you allow HeartVoice to release your Electronic Health record which includes Personal Data and Sensitive Personal Data to Integrated Wellness Clinic Pte Ltd, a Healthcare Provider.

“Electronic Health Record” means your account in our cloud-based storage holding your Personal Information and your Consultation record in accordance with the Ministry of Health’s guidelines on the retention of Patient Data.

“Personal Data” means the definition given to it in the PDPA including any information relating to an identified or identifiable natural person (“Data Subject”). An identifiable natural person is one who can be identified, directly or indirectly, in particular, by reference to an identifier such as a name, address, contact number, an identification number, an online identifier or to one or more factors specific to the physical, physiological, genetic, mental, economic, cultural or social identity of that natural person.

“Sensitive Personal Data” means any data which may reveal any racial or ethnic origin, political opinions, religious or philosophical beliefs, trade-union membership; data concerning health or sex life and sexual orientation; genetic data or biometric data; medical history, symptoms, complaints, allergies and medications

“Healthcare Providers” means any registered healthcare professional providing HeartVoice Services through its Apps and Websites including any persons/entities registered under the Private Hospitals and Medical Clinics Act Cap 248 (“PHMC”) as registered general practitioners;

  2. Please note that HeartVoice and Integrated Wellness Clinic Pte Ltd are independent companies.

B.  Coupon Details

(a)  S$ 30 Off any OMRON Monitor Purchase

  • Valid for 6 months from the date of purchase of the Programme
  • Each Coupon is applicable to one device purchase at a time
  • The coupon is only applicable for use at our HeartVoice Marketplace which you can access on the HeartVoice mobile application.
  • The Coupon is not transferrable and is not redeemable for cash and cannot be combined with any other coupons or any other offer or discounts or promotions offered by HeartVoice.

(b)  Free Telemedicine Consultation with HeartVoice Doctors

  • Valid for 6 months from the date of purchase of the Programme
  • Applicable for teleconsultation only with HeartVoice telemedicine providers listed in the coupon description
  • Medicine and delivery fees are charged separately and are not covered by this coupon
  • This coupon is not transferable and is not redeemable for cash and cannot be combined with any other coupons or any other offer or discounts or promotions offered by HeartVoice.

(c) Health Screening 

  • Valid for 6 months from the date of purchase of the Programme
  • The Health Screening Coupon can only be redeemed at Integrated Wellness Clinic located at 8 Sinaran drive #05-04, Novena Specialist Centre, Singapore 307470 on an appointment basis only.
  • Important: To redeem please present this coupon to Integrated Wellness staff at the clinic.
  • This coupon is not transferable and is not redeemable for cash and cannot be combined with any other coupons or any other offer or discounts or promotions offered by Integrated Wellness Clinic.
  1. Validity and Redemption
    1. All Coupons are valid for 6 months from the date of purchase of the coupon, after which the coupon will expire. TheCoupons will expire at 11:59:59 PM SGT on the date shown on the Coupon.
    2. The Coupon is denominated in Singapore Dollars.
    3. Coupons are intended for single use only.
    4. Coupons can be redeemed on the HeartVoice Mobile application.
    5. Coupons may only be redeemed towards the purchase of eligible products and /or services on the HeartVoice Mobile application.
    6. If a purchase exceeds the Coupon value, the remaining amount must be paid by the user. Unused coupons or any balance in a coupon may not be transferred to anyone else.
  1. Coupon Usage
    1. The Coupon is to be redeemed before the expiry date, failing which it will expire.
    2. Enter and apply the redemption code for your Coupon in the checkout where indicated before you make your purchase.
    3. The Coupon will be subject to the terms and conditions of specific campaigns run by HeartVoice from time to time.
    4. Delivery fee and / or Postage Costs (where applicable) and taxes are excluded from the Coupon discount.
    5. The Coupon is not legal tender and cannot be exchanged for cash.
    6. There shall be no cancellation of services or return of product obtained through the Coupon redemption.
  1. HeartVoice’s Rights
    1. HeartVoice reserves the rights to disallow or reverse the Coupon usage in the circumstances where HeartVoice believes that you have provided false information and /or manipulated or tampered with any part of the coupon.
    2. HeartVoice shall not be responsible for any claim, demand, loss, damage, expense or liability of any kind whatsoever, arising from or in connection with unauthorised use of this Coupon.
    3. HeartVoice reserves the right to cancel/amend or revoke the Coupon at any time.
